We at Periodontal Surgical Arts, believe in the overall welfare of our patients but also advocate doing our part to prevent the spread of COVID-19. Following the mandate from Governor Abbott and the Texas Board of Dental Examiners, we are following distancing guidelines. This includes minimizing the number of patients and team members in the practice at one time. Also, patients are being treated one at a time on a personalized basis. We do require that patients wear face masks and screen themselves prior to visiting. Thank you for your understanding.

First Visit

What will happen at my first visit?

Please assist us at the time of your initial visit to the office by providing the following information:

•	Your referral slip and x-rays from your referring dentist.
•	A list of medications you are currently taking.
•	Click here to print your forms & fill out prior to appointment. (or arrive 15 minutes early to complete in our office.)

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension.)

We will complete an in depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, TMJ and other signs of periodontal disease.
